If you lost your alcohol awareness card or TAM Card you need to go back to the office from which you originally received the card that you lost. You cannot go to any location as each location is a separate company. Unfortunately, there is no central searchable database of alcohol cards in Nevada. If you do not remember where you received your card, you will have to re-register and get a new card.
